2013 Registration Forms for Camp

The Personal Information and Session and Financial Agreement forms should be used to register for the Summer Program. These two forms can be filled out online or can be downloaded, printed out, completed and mailed to the address below.

The Directions for Administering Medication Form must accompany any medication sent or brought to camp. Please download, print out and complete this form.
